Honour it quickly. CAN-SPAM sets a limit of 10 business days, and other regimes expect it sooner. Keep an unsubscribed address on a suppression list so it is never mailed again, including after you import a new list.

Re-importing an old list can bring unsubscribed people back. Compare against your suppression list before verifying and sending.
